Accident Investigation

The purpose of accident or incident investigation is not to find out who or what is to blame. Accidents and incidents are investigated to prevent their recurrence.

At Matrix Safety Systems, we provide an organisation with full investigative reports for any accident or incident. We also offer professional recommendations to prevent the accident from occurring again.

Matrix Safety Systems Director, Warren James, was employed
for many years by Qantas Airways. He is a full member of the International Society of Air Safety Investigators (Australian
branch – ASASI).

Warren spearheads our team when investigating an accident or incident:

• We identify all contributing factors;
• Prioritise remedial actions;
• Provide high quality recommendations; and
• Promote improvements through and with your organisation’s OH&S Management System, such as developing a stronger safety culture.
